Tiara Johnson
Hey there,
I am a heart transplant recipient, Community Health Worker, and Board Certified Patient Advocate dedicated to supporting individuals and families living with heart disease and chronic illness. My work is shaped by both professional training and lived experience. After years of navigating heart failure, medical dismissal, and the transplant process, I recognized how often patients feel overwhelmed, unheard, and underprepared to manage their health. I created this space to help patients feel informed, supported, and more confident in understanding their health.
Through monthly support groups, educational webinars, and community workshops, I focus on assisting patients to better understand their diagnoses, communicate effectively with healthcare providers, and develop practical self-management skills they can apply in everyday life. My goal is to make health education accessible, honest, and empowering without overwhelming people.
In addition to support groups, I host workshops and webinars focused on chronic disease education, patient advocacy, health equity, and the social factors that influence health outcomes, particularly in underserved communities.
I believe that when patients understand their health, they are better equipped to advocate for themselves, make informed decisions, and feel less alone in the process.
